How to Enroll for Services

To Begin the “Divorce and Children” parent education class:

.

1. Download and print the Divorce and Children Enrollment form.
   .
2. Fax the completed form to (405) 364-1433 or turn it in to our front office. Payment is required at the same time you turn in the form.  The Divorce and Children class is $50 per person and we accept exact cash, money order, or credit card (Visa, Mastercard, and Discover).
